Initiated in 2005, one21 is an individual, independent Life Skills Program where the participants work closely with their case managers to set life goals and decide how best to achieve them. This becomes theirPErson Centred Plan (PCP). Participants have the option of participating in group activities and aspects of current programs or creating their own. One21 is designed to transition individuals into the adult world. If funded by the Department of Human Services DHS participants have three years to achieve this. However, many people may achieve this earlier.
one21 is ideal for young adults with a disability who are already quite
independent. Whether participants need full or part time
support, one21 is flexible enough to change with them and provide
just the right amount of support to help them reach their goals.
one21 case managers are skilled in assisting young adults to plan and to gain life skills for life's thrills. Examples of goals include:
Together, the participant and case manager find activities, classes, excursions or investigate/develop alternative programs that best suit the participant's needs.
Fee for service can be arranged for those individuals not funded by the DHS.
